ADAA Home Study Radiology Courses

Indiana requires Dental Assistants taking x-rays to be licensed by the Indiana State Department of Health (ISDH).  ISDH has approved six ADAA online courses to help candidates meet certification requirements.  These courses are of no additional cost to individuals who are Professional Members of the ADAA. Click here to view the membership application.  This program is not available to non-members.

While completing the required coursework, the Dental Assistant must apply for a “Provisional Permit”.  The online Provisional Permit Application can be found on the Indiana State Department of Health website. Follow the step-by-step instructions offered below.

Steps to Complete the Indiana Radiology Program

Step 1:  Apply to be an ADAA Professional member if you aren’t already.  Membership will give you online access the required courses listed above.  Within one business day of your membership confirmation, you will have access to the ADAA e-Learning Center to begin your coursework. 

Step 2: Complete and print the “Application for Provisional or Student Permit” on the ISDH website Indiana State Department of Health website.

  1. DO NOT add your Social Security Number to the application.
  2. List ADAA Online Study Courses on the application under the name of the school/program.
  3. List the address as 529 14th St NW, Suite 1280, Washington, DC, 20045, USA.
  4. Add the date you started the coursework and the anticipated date of completing the coursework.
  5. Mark “Provisional-Dental“, NOT Student on the application.
  6. Email the completed application to education@adaausa.org for the program director’s signature and date. Then the ADAA’s Director of Education, Holly Lundgren, will sign and date the form.

Step 3: When ADAA returns the signed application (with Holly Lundgren’s signature and date)

  1. Print it and add your Social Security Number
  2. Mail the printed, signed application to the Indiana State Department of Health (address on the form)
  3. Please keep in mind, if the permit application is not completed properly and does not have Holly Lundgren’s signature and date on it, the permit will not be processed by the Indiana State Department of Health.

Note:  You will be issued a Provisional Permit which means you can take x-rays in the dental office under the supervision of the dentist to complete clinical training. The Provisional Permit is good for 6 months from the date of issue. IT CAN NOT BE RENEWEDDental assistants are required to have an active provisional permit or an active Indiana Dental Radiographer license to take x-rays legally in Indiana.

Step 4: Complete all six radiology courses and pass each test.  Download each Certificate of Completion. If you have questions about the courses and printing certificates, please contact ADAA at info@adaausa.org.

Step 5: After passing the six courses and completing clinical training, please have the supervising dentist complete and sign the “Application for Proficiency Certification for Limited Radiographer” form on the ISDH website https://www.in.gov/health/cshcr/medical-radiology-services/.

Step 6: Register for the Dental Assisting National Board (DANB) Radiation Health and Safety (RHS) exam at www.danb.org. The DANB handles all questions about the exams. If you have passed the RHS exam previously, you do not need to retake it. You must have a copy of the official DANB certificate / letter showing the RHS exam was passed.

Step 7: Once the DANB RHS exam is passed and the official DANB certificate / letter is received (not preliminary pass letter), apply for the Indiana Dental Radiographer License.  Complete and print the “Application for Radiology License” on the ISDH website https://www.in.gov/health/cshcr/medical-radiology-services/.

  1. Mail the completed Application for Radiology License, license fee ($60 check or money order), along with the six Certificates of Completion from the ADAA, and copies of the official DANB certificate / letter and the Application for Proficiency Certification form signed by the dentist.  Mail all of the documents to the address on the Application for Radiology License.
  2. Keep copies of all the certificates of completion, official DANB certificate / letter, Application for Proficiency Certification, and Application for Radiology License for your records.
